We currently accept the insurances:

A $100 fee will be assigned to you for cancellations less that 24 hours in advance of your scheduled appointment.

If you are more than 15 minutes late for your scheduled appointment, you will need to be rescheduled and you will be charged a no-show fee of $100

If you do not show for your scheduled appointment, you will be charged a fee of $100. 

If you refuse testing in the office, you will be charged $100 cancellation fee.

If a check is returned for insufficient funds, you will be charged a $50 Returned Check Fee.
